VOLUNTEERS MUST COMPLETE AND SIGN THE WAIVER AND RELEASE FORM TO BE ELIGBLE TO SERVE WITH FRESH START FURNISHINGS.

This Release and Wavier of Liability (the “Waiver”) executed on _____________ (date) by _________________________ (“Volunteer”) in favor of Fresh Start Furnishings by Found in Faith Ministries, a nonprofit organized and existing under the laws of the State of Maryland, and each of its Directors, Officers, Employees, Advisors and Agents (collectively “Organization”).

 

Volunteer desires to provide volunteer services for Organization and engage in activities related to serving as a volunteer. Volunteer freely and voluntarily, without duress, executes this Waiver under the following terms:

 

  1. Waiver and Release: Volunteer releases and forever discharges and holds harmless Organization and its successors and assigns from any and all liability, claims, demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from the services Volunteer provides to Organization. Volunteer understands and acknowledges this Waiver discharges Organization from any liability or claim Volunteer may have against Organization with respect to bodily injury, personal injury, illness, death or property damage that may result from the services Volunteer provides to Organization occurring while Volunteer is providing volunteer services.

  2. Compensation: Volunteer understands that scope of Volunteer’s relationship with Organization is limited to a volunteer position and that no compensation is expected in return for services provided by Volunteer; that Organization will not provide any benefits traditionally associated with employment to Volunteer; and that Volunteer is responsible for his/her own insurance coverage in the event of personal injury, illness or property damage as a result of Volunteer’s services to Organization.

  3. Insurance: Further Volunteer understands that Organization does not assume any responsibility for or obligation to provide Volunteer with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance. Volunteer expressly waives any such claim for compensation or liability on the part of Organization beyond what may be offered freely by Organization in the event of injury or medical expenses incurred by Volunteer. 

  4. Assumption of Risk: Volunteer understands that the services provided to Organization may include activities that may be hazardous to Volunteer including, but not limited to driving delivery vehicle, lifting and moving heavy objects, and other services involving inherently dangerous activities. Volunteer expressly assumes risk of injury or harm from volunteer activities and Releases Organization from all liability. 

  5. Photographic Release: Volunteer grants and conveys to Organization all rights, title, and interests in any and all photographs, images, video or audio recordings of Volunteer or Volunteer’s likeness or voice made by Organization in connection with Volunteer providing volunteer services to Organization.

  6. Other: Volunteer expressly agrees this Waiver is intended to be as broad and inclusive as permitted by the laws of the State of Maryland and that this Waiver shall be governed by and interpreted in accordance with the laws of the State of Maryland. Volunteer agrees that in the event any clause or provision of this Waiver is deemed invalid; the enforceability of the remaining provisions of this Waiver shall not be affected.

 

By signing, I express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily.
